Identifying data:Ronald David Hanson (Ron) dob 4/6/1988 (age 15 years) brn/blue
Chief complaint:
Subject placed in The Program prior to detailed evaluation. Family Hx
issues self-reported by subject in soc/bio class, reported by soc/bio
instructor (Ms Foster). Subject’s behaviour tightly
self-controlled, and anomalous for a 15yo male in that he displayed
few erections during the first day of The Program.
Hx: None
Past Psychiatric Hx: No prior evaluations
Past med/surg Hx: See Family Hx
Current Meds: None Rx, none otc
Drug Allergies: None identified
Med Review: Healthy 15yo male.
Substance Use Hx: None identified
Family Hx:
Source: Newspaper accounts, briefing by caseworker, numerous, voluminous court, caseworker files. Biological parents divorced at age 1. Mother remarried at age 2. Abuse by step-father. Numerous burns from cigarette rt upper chest and back age 3 reported by neighbor. Abuse confirmed but paradoxically CPS did not remove child, even after reporting neighbor obtained restraining order against threats by step-father. Mother and step-father produce daughter. Step-father beats mother (died q 2 weeks), daughter (doa), beats subject requiring partial splenectomy. Subject sent to live with aunt and uncle in Japan (age 4 1/2). Step-father incarcerated (life/no parole). Families of subject’s biological parents continue to battle in court. Court recently reversed custody, awards custody to mother’s family, who place subject with aunt and uncle in this city. Subject was removed from home (sic?) environment in Japan to this area on no notice and with little or no explanation.
Psychosocial Hx:
Arriving at a proper psychosocial history and diagnosis is complicated by cultural barriers. While subject has a superb command of spoken and written English (20+ yo equiv), this is overlaid by Japanese culture. Working with subject’s Japanese language instructor, I understand that behaviour which may be deemed “blunt aspect” or withdrawl in American society is considered polite respect for elders/superiors in Japan. In Japan, avoiding direct eye contact is a sign of respect when in the presence of superiors.
Japanese concepts of Honne and Tatamae quite relevant here. Tatamae is the “public” face one shows, which must always be under control and in line with social/societal norms. Honne is the “private” face seen by few, if any. Subject has been given few, if any, outlets for emotion and/or anger.
Subject was in abusive environment ages 2 through 4 1/2. Aunt and uncle in Japan are Mormon missionaries, providing strict environment. Subject immersed in Japanese culture, language, education system, and home-schooled in English; aunt teaches English in local Japanese school.
Cultural differences make it difficult to differentiate social skills, which subject possesses at a level at least age-appropriate if not advanced to the 20yo level, from personal and inter-personal skills, which may be underdeveloped. Particular focus on personal skills for anger management; developing, maintaining, relying on emotional support network.

Mental Status:
Folstein: 30/30
Standard suite (given prior to school entrance) shows subject to be in elevated/gifted range in all categories. Anger, resentment, self-image issues?
Physical:
Physical Education staff consider subject to be above average in
coordination, and very athletic in swimming and track. Difficulty
with team sports, but it is unclear whether this stems from
socialization issues, or from lack of familiarization with American
team sports.
Differential diagnosis/treatment plan:
Continue evaluation/monitoring. This is one tough kid! Anger (rage?) shows through his mixed-culture basis occasionally, but unclear if this is significantly different from nominal 15 yo issues.
