If you're careful, you should have great fun, and not need medical attention.
The first time you try heel trampling, ask the girl to use a thick/wide heel. Get a feel for the weight and pressure that a heel concentrates. It's quite a different feeling from bare feet, which give more of a generalized pressure. Also, with a heel the girl needs to be more careful with her balance and movements; if she leans backward with her weight, you will feel it acutely. Until you both get comfortable, she should try to keep her weight forward on the balls of her feet. Try to avoid the heels too high up on your chest (shoulder joint), or at the bottom of your sternum (breastbone), which in theory could snap off. If she's on your back, have her try to avoid heels on your spine or kidneys.
Once your okay with wide heels, try the ultimate: high heels. Accept the fact that your skin may be covered with heel marks and maybe cuts afterward. Clean the shoes off with alcohol first. Make sure that the heels are not brand new; if so, they may be very sharp at the edges and need a little filing first. It's never a bad idea to check the heels anyway, even if worn, for sharp edges, metal, etc. High heels do cause significant pain, so expect it, and you'll be able to take it better. I recommend that the heel point be at least 1 cm. or 1/2 inch in diameter, or else she probably won't be able to stand full weight normally on your chest and stomach. The major determining factor for pain and tolerance is not the height of the heel, but the diameter of the point which rests on your skin, and whether it is smooth or sharp. (Of course, the weight of the girl also matters!) Have a safeword for her to get off quickly should something go wrong the first few times you try it. Also, keep communicating with each other during the trample until you both are comfortable with high heels and know what to expect.
